• Over 38 kg (85 lbs): 250-400 mg per day, in two divided doses.

Patients 10 and up:

• Week 1, start with 50 mg per day, divided into two 25 mg doses, taken morning and night.
• Week 2, switch to 100 mg per day, divided into two 50 mg doses.
• Week 3, switch to 150 mg per day, divided into two 75 mg doses.
• Week 4, switch to 200 mg per day, divided into two 100 mg doses.
• Week 5, switch to 300 mg per day, divided into two 150 mg doses.
• Week 6, switch to 400 mg per day, divided into two 200 mg doses.

Other medications are commonly given during treatment with Topamax sprinkles, and this can significantly influence dosage. Doctor instructions should always be followed.

Off-label, the medication can also be used for cluster headaches; instructions are the same as for migraines except the ultimate daily dose is 200 mg per day, taken in two 100 mg doses. Dosage increases should be no higher than 25 mg per week.

It takes about two months for full effects to be felt when used for seizures; those who buy Topamax Sprinkle for migraines may need to wait three months.
